Home Based Team

The Home Based Team (HBT) provides highly individualized supports and services to help strengthen youth and their families as they struggle to reduce child protective concerns, build resiliency, become empowered, and seek self-sufficiency.

Home Based Team provides child-centered, family-focused, and culturally competent services to youth who experience serious behavioral and mental health challenges, and their parents. Services may be provided in various settings, including the family’s home, the Home Based Team office, or a community-based setting.

The goal of the Home Based Team is to work collaboratively with parents, youth and their treatment team to establish a vision, strengths, goals and expected outcomes for services offered to youth and families. Our staff includes therapeutic mentors, clinicians and parent professionals.

The Home Based Team is a program of BAMSI’s Children, Family and Community Services Division that integrates the principles of Wraparound; a philosophy of care that includes the youth and family in every stage of the planning process. Services are individualized and strength-based and are geared toward reaching a positive set of outcomes.

  • Comprehensive home-based assessments
  • Therapeutic mentoring: Clinical and non-clinical
  • Intensive In-Home therapy
  • Comprehensive support and stabilization
  • Team Meeting Facilitation
  • Hospital discharge meetings
  • Parenting assessments
  • Respite services
  • Child and youth support
  • Therapeutic recreation
  • Parent and family support
  • Family Resource Center

HBT services may be accessed through the Department of Child and Family Services (DCF), the Community Service Agency (CSA) or through MassHealth.

BAMSI’s Home Based Team program is located at 300 Battles Street, Brockton.
