BAMSI VOLUNTEERS

BAMSI’s Executive Management Team (EMT) members

Don Zimmerman and Pat Algird participated in holiday preparations with Easton-based, My Brother’s Keeper who distributes furniture during the calendar year and gifts at the holiday season. EMT members worked side by side with community volunteers the morning of December 18, 2009 wrapping presents for delivery to one of the over 2,000 families from over 60 communities who receive donations during the holiday season.

Case Management for Individuals and Families Infected and Affected by HIV

Monday to Thursday – 8:00 a.m. to 5:00 p.m.
Friday – 8:00 a.m. to 4:30 pm.

Program Description:

The Case Management program serves HIV infected adults, children, adolescents, and their families providing access to medical, social, and support services to help individuals better manage their health and well-being and improve their overall quality of life. Today, HIV is no longer considered a terminal illness, but rather a manageable chronic health condition. With essential supports and encouragement, people with HIV can continue to live full lives. Case Management serves individuals of all ages and diverse backgrounds and provides a welcoming and affirming environment for individuals and families coping with the many challenges associated with HIV. Case Management also works closely with the Brockton Neighborhood Health Center and area physicians to coordinate health care and other services. Case Management serves approximately 300 individuals and family members annually.

Case Management provides:

  • Individual needs assessments;

  • HIV antibody counseling and testing;

  • Case management (including coordination of care plan);

  • Education about HIV;

  • Advocacy for medical, financial, and legal needs;

  • Access to medications and clinical trials;

  • Nutrition assistance;

  • Home health services;

  • Referrals for home health care, mental health counseling, and support groups; and

  • Social events and activities.

Case Management’s bilingual/bicultural staff is educated about and sensitive to the special issues and needs of people living with HIV. Staff is also aware of the many challenges that people living with HIV face on a daily basis such as language barriers, lack of financial resources, lack of information, fear, prejudice, and discrimination.

Eligibility for Services:

Individuals with a documented HIV diagnosis and their families from Brockton and surrounding towns are eligible for services.
