Home Based Team (HBT)Children, Youth, and Family Services
The Home Based Team (HBT) engages youth and families by providing needed supports and highly individualized services in order to help strengthen families as they progress toward their goals, reduce child protective concerns, and encourage resiliency, empowerment, and self-sufficiency. HBT services are available to youth who experience serious behavioral and mental health issues and their parents. The HBT staff work collaboratively with the family and their treatment team to establish goals for service and expected outcomes.
All services are child-centered, family-focused, and culturally competent. Services may occur in a family’s home, the HBT office, or in a community-based setting. The HBT philosophy promotes families as the most important caregivers of their youth, and therefore critical in the planning and implementation process. The HBT staff work together with parents and youth to identify strengths, which are emphasized during the intervention process.
HBT services may be accessed through the Department of Mental Health (DMH), the Department of Child and Family Services (DCF), the Community Service Agency (CSA) or through MassHealth. For more information about the Home Based Team, please call 508-584-0074.
