Nursing Home OutreachDay and Residential Services

Nursing Home Outreach staff provides services within nursing facilities to individuals with developmental disabilities and brain injury.  Services include day habilitation and transitional services.  A series of planned, goal-oriented supports is designed to maximize each individual’s potential.  This includes training in self-care, communication, social skills, sensory-motor development, safety, community independence, and activities of daily living.  To promote community integration, individuals are supported and encouraged to volunteer, use community resources, develop travel skills, and join community organizations.  Individuals also have access to therapeutic supports, such as physical therapy, occupational therapy, speech and language therapy, behavior management, group therapy, individual counseling, and recommendations for and use of assistive technology.

To be considered eligible, individuals must be at least 22 years old, have a developmental disability or brain injury, and have been screened through the Pre-Admission Screening and Resident Review process and determined to require specialized services.  For more information, contact one of the Nursing Home Outreach sites:
